Cannabis for diabetic neuropathy: evidence and clinical assessment
Painful diabetic nerve damage needs individual assessment. Small cannabis studies establish neither a general treatment recommendation nor a replacement for diabetes care.
TL;DR: Results are mixed: a small inhaled-THC trial examined short-term pain effects, while a nabiximols study found no significant difference from placebo. Neither establishes CBD as standard treatment or a personal dose. Discuss pain treatment and diabetes management with the relevant clinicians.
What the studies actually show
In 2015, Wallace and colleagues studied inhaled cannabis in 16 people in a small controlled trial. Observed short-term pain effects do not establish long-term benefit or suitability for everyone with diabetic neuropathy.
In 2010, Selvarajah and colleagues found no significant difference between nabiximols and placebo in the primary pain comparison in a trial of 30 people. It should not be presented as a positive demonstration of superiority.
Results across different neuropathic conditions, products and study durations are not interchangeable. They do not establish a general success rate applicable to your case.
No standard dose from a product page
Any proposed treatment needs individual medical assessment. Do not replace a prescription with a general CBD or THC dose. Do not independently stop insulin, other diabetes medicines or pain medication.
Have the actual product and complete medication list reviewed. A blanket assurance of no interaction with insulin or metformin cannot replace that assessment. Freely sold CBD oil does not demonstrate an effective treatment for diabetic nerve pain.
Diabetes management remains a separate task
Pain scores and glucose patterns answer different questions. Agree goals, reviews and a personal sick-day and emergency plan with your diabetes team. Pain relief does not establish improvement of the underlying diabetes.
Vomiting, unexpected readings or confusion need appropriate assessment rather than being automatically attributed to cannabis. This also applies when a product was prescribed for another condition.
With diabetes, blood glucose, medicines and warning signs also need individual consideration. Cannabis with diabetes: blood sugar and everyday care

FAQ
Does cannabis replace my gabapentin?
These studies cannot determine that for your case. Any change to pain treatment must be agreed with your treating clinician.
Is CBD standard treatment for diabetic neuropathy?
This page does not establish sufficient evidence for that claim. Marketing and individual studies cannot replace a personal treatment recommendation.
Does membership guarantee a prescription or reimbursement?
No. Prescribing, actual costs and insurance coverage must be clarified independently of membership.
