PTSD and cannabis: hope meets data gaps

Cannabis is not an established treatment for post-traumatic stress disorder. The US VA/DoD guideline from 2023 recommends against it because of insufficient efficacy evidence and risks. This is an international clinical position, not Swiss law. Individual reports of better sleep or fewer nightmares do not establish treatment of PTSD as a whole.

TL;DR

  • Cannabis is not an established treatment for post-traumatic stress disorder. The US VA/DoD guideline from 2023 recommends against it because of insufficient efficacy evidence and risks. This is an international clinical position, not Swiss law. Individual reports of better sleep or fewer nightmares do not establish treatment of PTSD as a whole.

What the research actually shows

Available studies do not support a reliable general recommendation. Observed improvements can have several causes and are not automatically a demonstrated medicine effect. A plausible mechanism involving REM sleep is not a substitute for clinical evidence. A blanket claim that cannabis prevents long-term emotional processing would also overstate what is known.

What this means for treatment

Care should focus on treatments suited to PTSD, particularly evidence-based trauma-focused psychotherapy. Sleep, re-experiencing, avoidance and heightened arousal need separate assessment. People already using cannabis should be able to discuss it without shame. An appointment can clarify which symptoms are being targeted and which might relate to use or changes in use.

Risks and questions for your appointment

THC can worsen anxiety, panic or suspiciousness in some people. Describe immediate relief alongside effects on daily life, concentration and treatment. Problematic use calls for support, not leaving PTSD untreated. An acute mental health crisis or immediate danger to yourself requires urgent help; in Switzerland call 144 or 112.

Swiss rules and reimbursement: separate assessments

Since August 2022, medical prescribing of cannabis medicines no longer requires an exceptional FOPH permit. This does not establish authorisation for every condition or automatic reimbursement. The preparation, indication and applicable reimbursement rules need separate assessment.

Diagnostic records, previous treatment attempts and an up-to-date medication list are more useful for an appointment than a preferred product name. Record the everyday difficulties you hope to improve, remaining uncertainties and who will monitor outcomes and tolerability. Club membership does not replace medical assessment.

FAQ

Are fewer nightmares enough to establish treatment of PTSD?

No. One symptom does not represent the whole disorder. Other symptoms, everyday functioning, adverse effects and longer-term outcomes also matter.

Does a prescription guarantee reimbursement?

No. Prescribing, medicine authorisation and reimbursement are separate questions. Clarify which rules apply to the preparation and indication, including whether prior reimbursement approval is needed.
