Chronic pain: what cannabis really delivers

Cannabis is not an established treatment for every type of chronic pain. The 2026 Cochrane review rates evidence for neuropathic pain as low to very low certainty. Whether a particular medicine is appropriate requires clinical assessment of the diagnosis, previous treatments, everyday limitations and risks.

TL;DR

  • Cannabis is not an established treatment for every type of chronic pain. The 2026 Cochrane review rates evidence for neuropathic pain as low to very low certainty. Whether a particular medicine is appropriate requires clinical assessment of the diagnosis, previous treatments, everyday limitations and risks.

What the research actually shows

The updated review concerns chronic nerve pain. Its uncertain findings justify neither a general promise of benefit nor the claim that nobody can benefit. An older statistic from the 2018 version does not automatically represent current evidence. Back pain, inflammatory pain and neuropathic pain should not be treated as interchangeable research populations.

What this means for treatment

A useful treatment should make a meaningful difference to daily life: movement, sleep or participation in valued activities. A lower pain score alone does not answer every question. Before an appointment, record previous treatments, unwanted effects and the goals that remain unmet. The decision concerns a specific preparation rather than cannabis as one uniform product.

Risks and questions for your appointment

Discuss drowsiness, falls, mental health symptoms and interactions with your full medication list. Agree how benefits and burdens will be assessed and when treatment will be reconsidered. A THC:CBD ratio or dose found online is not an individual treatment plan. Do not independently replace or stop other pain medicines.

Swiss rules and reimbursement: separate assessments

Since August 2022, medical prescribing of cannabis medicines no longer requires an exceptional FOPH permit. This does not establish authorisation for every condition or automatic reimbursement. The preparation, indication and applicable reimbursement rules need separate assessment.

Diagnostic records, previous treatment attempts and an up-to-date medication list are more useful for an appointment than a preferred product name. Record the everyday difficulties you hope to improve, remaining uncertainties and who will monitor outcomes and tolerability. Club membership does not replace medical assessment.

FAQ

Does uncertain evidence mean cannabis never works?

No. It means research cannot estimate benefits and harms with sufficient confidence. An individual testimonial does not resolve that uncertainty, and neither does a blanket dismissal. A reasoned clinical benefit–risk assessment remains essential.

Does a prescription guarantee reimbursement?

No. Prescribing, medicine authorisation and reimbursement are separate questions. Clarify which rules apply to the preparation and indication, including whether prior reimbursement approval is needed.
