MS spasticity: cannabis' place in Swiss therapy
In multiple sclerosis, nabiximols may be considered for certain adults whose spasticity is inadequately controlled. Authorisation for spasticity is not authorisation to treat MS progression. The symptom being targeted, eligibility and monitoring of benefit belong within neurological care.

What the research actually shows
Cochrane assesses short-term improvement in patient-reported spasticity with nabiximols more favourably than many other cannabis uses. This does not establish improvement across all MS symptoms. Perceived relaxation, measured mobility and quality of life are distinct outcomes and should not be presented as interchangeable evidence.
What this means for treatment
Sativex is authorised in Switzerland for a defined spasticity setting. Current prescribing information, previous antispastic treatments and individual response matter. Discuss concrete goals such as personal care, night-time symptoms or movement. Disease-modifying MS therapies serve a different purpose and are not replaced by symptom treatment.
Risks and questions for your appointment
Dizziness and drowsiness may add to existing mobility difficulties. Clarify which changes count as benefit and which adverse effects would prompt reconsideration. Dosing and monitoring depend on the specific medicine and treating specialist. A universal spray schedule found online is inappropriate.
Swiss rules and reimbursement: separate assessments
Since August 2022, medical prescribing of cannabis medicines no longer requires an exceptional FOPH permit. This does not establish authorisation for every condition or automatic reimbursement. The preparation, indication and applicable reimbursement rules need separate assessment.
Diagnostic records, previous treatment attempts and an up-to-date medication list are more useful for an appointment than a preferred product name. Record the everyday difficulties you hope to improve, remaining uncertainties and who will monitor outcomes and tolerability. Club membership does not replace medical assessment.
FAQ
Does cannabis slow the progression of multiple sclerosis?
Possible relief of spasticity does not demonstrate slower disease progression. Symptom management and disease-modifying treatment need separate assessment.
Does a prescription guarantee reimbursement?
No. Prescribing, medicine authorisation and reimbursement are separate questions. Clarify which rules apply to the preparation and indication, including whether prior reimbursement approval is needed.
