Cannabis and sleep: what studies actually show

CBD is not an established standard treatment for persistent insomnia. A small randomised trial in 2024 found no advantage over placebo on most sleep measures. Feeling drowsy after cannabis is also not the same as getting restorative sleep. Assessment should address the cause of sleep difficulties and their daytime consequences.

TL;DR

  • CBD is not an established standard treatment for persistent insomnia. A small randomised trial in 2024 found no advantage over placebo on most sleep measures. Feeling drowsy after cannabis is also not the same as getting restorative sleep. Assessment should address the cause of sleep difficulties and their daytime consequences.

What the research actually shows

The CBD trial was small and short. Some favourable findings therefore do not establish sustained effectiveness for insomnia. Results from THC-containing preparations cannot simply be applied to CBD. Studies of sleep in people with pain also do not necessarily answer the question of a separate insomnia disorder.

What this means for treatment

Describe difficulty falling asleep, waking during the night and daytime sleepiness separately. Snoring, breathing pauses, shift work, alcohol and other medicines may matter to the assessment. Ask about treatments specifically addressing insomnia, including cognitive behavioural therapy for insomnia, rather than looking only for a sedating product.

Risks and questions for your appointment

Record next-morning alertness as well as hours slept. Discuss combinations with sleeping tablets, alcohol or other sedating substances. If cannabis use is regular, changes in use may also affect sleep and belong in the clinical history. An online dosing schedule cannot replace this assessment.

Swiss rules and reimbursement: separate assessments

Since August 2022, medical prescribing of cannabis medicines no longer requires an exceptional FOPH permit. This does not establish authorisation for every condition or automatic reimbursement. The preparation, indication and applicable reimbursement rules need separate assessment.

Diagnostic records, previous treatment attempts and an up-to-date medication list are more useful for an appointment than a preferred product name. Record the everyday difficulties you hope to improve, remaining uncertainties and who will monitor outcomes and tolerability. Club membership does not replace medical assessment.

FAQ

Is falling asleep faster enough to establish benefit?

No. Night-time waking, restoration, daytime functioning and adverse effects also matter. A short-term subjective effect does not establish lasting benefit or treatment of the underlying cause.

Does a prescription guarantee reimbursement?

No. Prescribing, medicine authorisation and reimbursement are separate questions. Clarify which rules apply to the preparation and indication, including whether prior reimbursement approval is needed.
