Cannabis & Women's Health Switzerland: PMS, Endometriosis, Menopause | CannabisClub.ch
How does cannabis affect women? Endocannabinoid system, cycle, PMS, endometriosis, menopause — plus Swiss legal status and interactions with pill and HRT.
Women experience cannabis differently than men — hormonally, metabolically and in effect. Yet women's health remains largely under-illuminated in the Swiss cannabis debate. This guide covers how the female endocannabinoid system interacts with the cycle, what studies say about PMS, endometriosis and menopause, which CBD/THC uses are legal in Switzerland, and the warnings (pregnancy, pill, breastfeeding) to observe.
Important: This article is not a substitute for medical advice. Cannabis (incl. CBD) is generally not recommended in pregnancy or breastfeeding. Consult your doctor for existing conditions or medications.
TL;DR — Cannabis & women's health Switzerland
- ECS & cycle: endocannabinoid activity fluctuates with estrogen (peak around ovulation)
- PMS/menstrual pain: CBD/full-spectrum can reduce pain intensity (user reports, small studies)
- Endometriosis: ECS dysregulation documented — medical cannabis with BAG approval possible
- Menopause: hot flashes, sleep, mood — CBD/CBN with indica terpenes
- Pill interaction: CBD inhibits CYP3A4/2C19 → contraceptive pill may be affected
- Legal Switzerland: CBD ≤1 % THC freely available, THC only via prescription / pilot project
Women & the endocannabinoid system
The endocannabinoid system (ECS) regulates pain, mood, sleep, appetite, immune response — and in women is hormonally modulated. Studies (Craft et al. 2013, Fattore & Fratta 2010) show estrogen increases anandamide activity, while progesterone lowers it. Practical consequence: cannabis can feel different across cycle phases.
Cycle phases & cannabinoids
- Follicular phase (day 1–14): rising estrogen → ECS more active, effect often stronger
- Ovulation: anandamide peak, high receptor sensitivity
- Luteal phase (day 15–28): progesterone dominates, more sleep issues & PMS
- Menstruation: prostaglandin-triggered cramps — CBD/full-spectrum act via CB1/CB2 & TRPV1
PMS & menstrual cramps
Premenstrual syndrome (PMS) and dysmenorrhoea affect up to 80 % of women. Full-spectrum CBD acts via multiple mechanisms: prostaglandin modulation, TRPV1 activation, serotonin modulation. Users report reduced cramp intensity and better sleep. Dose: individual titration — start with 10–20 mg CBD, 2× daily (see CBD dosing guide).
Endometriosis — evidence & CH reality
Around 190,000 women in Switzerland live with endometriosis. Studies (Sanchez et al., Bouaziz et al.) show ECS dysregulation in endometriotic lesions — CB1/CB2 are aberrantly expressed. THC and CBD inhibit endometrial cell proliferation and modulate pain in lab models.
In practice: medical cannabis can be prescribed for endometriosis in Switzerland — provided standard therapy is exhausted, with prescription and BAG approval. See prescription guide and insurance guide.
Perimenopause & menopause
Between the late 40s and mid-50s the hormonal balance changes fundamentally. Common symptoms: hot flashes, sleep disorders, mood swings, joint pain, libido loss. The ECS is involved in all of these — hence CBD is increasingly discussed as an adjuvant.
- Hot flashes: CBD may modulate via serotonin receptors
- Sleep: CBN-rich strains, indica-dominant genetics with myrcene/linalool
- Mood/anxiety: CBD via 5-HT1A
- Joints: topical CBD applications
Interactions (pill, HRT, antidepressants)
- Combined contraceptive pill: theoretically less effective ovulation protection with chronic high CBD use — consult gynaecologist
- Hormone replacement therapy: levels can be affected
- SSRIs: stronger effect possible — monitor
- Blood thinners (warfarin): INR monitoring needed

FAQ
Does CBD help with menstrual cramps? Evidence limited but positive; users report clear relief. Titrate individually, 10–20 mg CBD 2× daily to start.
Is medical cannabis approved for endometriosis in Switzerland? Yes, off-label with BAG approval when standard therapy fails.
CBD and contraceptive pill — safe? Usually fine at low doses. High chronic use: consult gynaecologist.
Which strain for menopause? Indica-dominant with high CBD and myrcene/linalool. Add CBN for sleep.
CBD in pregnancy and breastfeeding? No — generally advised against; data insufficient and CBD/THC cross the placenta.
Does health insurance cover it? In individual cases yes, with cost approval.
