What is cannabis-induced psychosis?

Psychosis means impaired reality testing — hallucinations (mostly auditory), delusions (persecution, reference) and disorganized thought. DSM-5 substance-induced psychotic disorder (F19.9) appears either acutely or as an extended episode over days to weeks. It differs from a normal cannabis high through impaired insight and persistent delusional content.

Evidence — what is the actual risk?

The EU-GEI study (Di Forti 2019) showed daily users of high-potency cannabis (>10 % THC) had up to 5× elevated risk of first psychotic episode. In Amsterdam (33 %) and London (30 %) daily high-THC use was the single strongest population-attributable factor.

THC dose, potency and setting

Higher dose + higher THC + higher frequency = higher risk. For at-risk individuals: stay under 10 % THC, choose CBD-rich strains, avoid concentrates/dabs, no edibles without experience (see the consumption methods guide).

Is CBD a protective factor?

CBD acts opposite THC in several ways: antipsychotic (McGuire 2018 phase-II RCT), anxiolytic, and it reduces paranoia during THC challenges. Balanced THC:CBD (1:1, 1:2) strains are notably safer for at-risk profiles.

  1. Calm environment, reduce stimuli.
  2. Trusted person present, don't leave alone.
  3. Don't argue with delusional content — validate and reassure.
