Cannabis & Psychosis — Clarifying the Risk
Cannabis does not cause psychosis in everyone. However, in cases of genetic predisposition, it can trigger an onset — especially with daily use of high-potency cannabis before the age of 18.
TL;DR: Marconi 2016: OR 3.9 for psychosis with daily high-potency cannabis use (>10% THC). Di Forti 2019: explains up to 30% of first-episode psychoses in London/Amsterdam. Andréasson 1987 (Swedish conscript cohort): causal contribution acknowledged. CBD shows partially protective effects (Bhattacharyya).
What 'Psychosis Risk' Actually Means
It does not mean that every use leads to psychosis. Rather, if there is a predisposition, cannabis can trigger or accelerate the onset.
The general lifetime risk for schizophrenia is ~1%. With daily use of high-potency cannabis, it rises to ~3–5%.
For the 15% of people with a family history, the personal risk is significantly higher.
Warning Signs — When to Stop
Paranoid thoughts that persist while sober. Delusions of reference and ideas of persecution.
Hallucinations occurring without the acute effects of cannabis.
Thought fragmentation and unprovoked social withdrawal.
If these symptoms occur: stop immediately and seek medical evaluation. In an emergency: call 144.
FAQ
Is it reversible?
Substance-induced psychosis often resolves within weeks after abstinence. Triggered schizophrenia is chronic but treatable.
Does CBD protect against it?
Bhattacharyya, Iseger 2015: high doses of CBD show partially antipsychotic effects. However, it should not be relied upon for self-protection — modern high-potency cannabis with low CBD content is particularly risky.
