Medical vs. recreational cannabis in Switzerland

Two completely separate legal frameworks — here is the clear comparison. Prescription, pilot project, Cannabis Club or CBD: what fits you?

Medical: narcotics prescription, diagnosis required, partial insurance coverage, dosed preparations.

Legal recreational: only via pilot projects (FOPH studies) or CBD (less than 1% THC).

Cannabis Club: private-use framework, no commercialisation — grey zone.

Medical THC content dosed (2–20%), recreational studies 5–20%.

Driving fitness: 1.5 µg/L THC blood threshold for both — do not drive.

Legal framework side by side

Switzerland has four clearly separated cannabis access routes:

  • Medical (NarcA art. 8 revised 2022): prescription, pharmacy, diagnosis
  • Pilot project (NarcA art. 8a since 2023): city studies in 12 cities, lottery
  • Legal CBD (less than 1% THC): free sale, no psychoactive effect
  • Cannabis Social Club: private community, no commercial sale

Access & availability

Medical: via your doctor, immediate start after prescription, unlimited while treatment lasts. Pilot project: only with residence in a study city (Zurich, Basel, Bern, etc.), admission by lottery, duration ~30 months.

Cannabis Club: private membership with age control and regulated drops — no public sale. CBD: free purchase in specialised stores, online and kiosks from age 18.

Preparations & THC content

Medical preparations have standardised active ingredient contents and batch analysis: Sativex® (2.7 mg THC + 2.5 mg CBD per spray), Epidyolex® (CBD 100 mg/ml), Bedrocan flowers (14–22% THC, defined terpene profiles), individual oils on prescription.

Pilot cannabis is supplied by certified producers (Pure Production, Sanaty Farm) for studies — usually 5–15% THC, lab-tested. Cannabis Club drops follow similar quality standards; CBD products by definition contain < 1% THC.

Cost & insurance

Medical: 400–900 CHF/month, partial coverage (see insurance spoke). Pilot project: subsidised prices (~8–12 CHF/g), no coverage. Cannabis Club: transparent membership and drop prices, example: 8–15 CHF/g. CBD: 5–20 CHF/g depending on quality.

Which for whom?

Medical: clear indication and doctor willing to prescribe, costs covered. Pilot project: residence in study city, willingness to participate in study (questionnaires, samples). Cannabis Club: private setting, no medical indication, focus on quality community. CBD: no psychoactive effect desired, everyday/wellness.

Can I use both at the same time?

No — pilot study conditions generally exclude parallel sources. Medical treatment and CBD are compatible if the doctor is informed.

Is the Cannabis Club legal?

Private consumption is decriminalised (<10 g fine). A purely private club with age control and no commercial sale sits in a grey zone but without known prosecutions when rules are respected.

Do I have to join the pilot project if I live in Zurich?

No — participation is voluntary, via application and lottery.

What about CBD flowers with high CBD content?

CBD flowers under 1% THC are freely available. Relaxing, not psychoactive. Therapeutic effect usually needs higher doses and medical supervision.

Cannabis access is legally complex and evolving. This overview is informational, not legally binding.
