Cannabis and health insurance in Switzerland

Cannabis medicines are expensive — 400 to 900 CHF per month. Basic insurance covers selectively under HIO art. 71a/b. Here is the real workflow and the most common denial reasons.

Sativex® is listed in the specialties list for MS spasticity — direct reimbursement.

Epidyolex® is listed for drug-resistant epilepsy (Dravet, Lennox-Gastaut).

Magistral preparations (THC/CBD oils, flowers) need prior authorisation under HIO art. 71a.

Approval rate 2024: ~65% with reasoned application, ~30% without documented prior therapies.

Application by the doctor, processing 2–6 weeks.

Legal basis HIO art. 71a/71b

The Health Insurance Ordinance (HIO/KVV/OAMal) regulates in art. 71a and 71b coverage for medicines in off-label use or not listed in the specialties list (SL). Cannabis magistral preparations fall under this rule.

Requirements: proven therapeutic benefit, no or insufficient alternative, reasonable cost-benefit ratio. The insurer's medical officer decides.

What basic insurance covers

Listed finished preparations are directly reimbursed (after deductible/co-payment):

  • Sativex® — multiple sclerosis spasticity
  • Epidyolex® — drug-resistant epilepsy
  • Marinol®/Dronabinol — sometimes listed for chemotherapy nausea

Prior authorisation — the real workflow

For all non-listed preparations, the doctor must apply for prior authorisation before treatment start.

The application includes diagnosis (ICD-10), prior therapies, current state, cost-benefit argument and study references. Success rate 2024: about 65% with detailed application.

What to do if denied

Denials are common for first applications without documented prior therapy or off-label indication without literature. Options: objection within 30 days, second opinion of the medical officer, application update, switch to listed alternative (Sativex® for spasticity).

Supplementary insurances (Complementa, ASSURA CompleTOP) sometimes cover off-label cannabis with noticeably higher approval rate.

Is Sativex® directly covered?

Yes, for MS spasticity after at least one unsuccessful prior therapy. Coverage after deductible and 10% co-payment.

What is the approval rate?

For detailed applications (therapies, studies, prognosis): about 65% (2024). Without prior-therapy documentation, notably lower.

Do I need to pay upfront?

For listed finished preparations: no. For magistral preparations without prior authorisation: yes, until the application is approved. Retroactive coverage possible but uncertain.

Does supplementary insurance cover cannabis?

Some insurers (Complementa, Sanitas Classic, Helsana Completa) partially cover off-label cannabis. Check policy details.

Coverage rules change. Check with your insurer and doctor for the current situation before starting treatment.
