CBD vs THC for pain — what actually works?
Not all pain is the same. CBD and THC act at different sites — and their combination often beats either one alone.
THC is pharmacologically stronger in acute and neuropathic pain (direct CB1 agonist). CBD is primarily anti-inflammatory, calming, and indirectly lowers pain perception. In chronic pain, the CBD + THC combination (entourage effect) shows the best outcomes. Mild muscle soreness, tension, tension headache: CBD often suffices. Chronic neuropathic pain, MS spasticity, cancer pain: medical cannabis with THC after consultation.
How CBD and THC act in the pain system
THC binds directly to CB1 receptors in the central nervous system and CB2 receptors on immune cells. This dampens pain signal transmission and reduces inflammation. Relatively fast and strong action.
CBD barely binds CB1/CB2 directly. Instead it inhibits the breakdown of endocannabinoids, activates TRPV1 and 5-HT1A receptors, and reduces pro-inflammatory cytokines. Subtler effect, no high, but noticeable with longer use.
Which type of pain — which compound
Acute muscle pain, tension, mild headache: CBD usually enough. Topical (balm) or oral (oil, flowers). Instantly available, legal, no prescription.
Inflammatory pain (osteoarthritis, mild arthritis, sports injuries): CBD is especially useful here due to its anti-inflammatory profile.
Evidence — short and honest
The NASEM review (US, 2017) rated the evidence for cannabis/cannabinoids in chronic adult pain as «substantial» — robust. Later reviews confirm benefit mainly in neuropathic pain.
For pure CBD the evidence is weaker. It acts more as an adjuvant — improving stress, sleep and inflammation and thereby lowering pain perception.
Practical path in Switzerland
Step 1: start with legal CBD for mild-to-moderate pain. Full-spectrum oil 10–15% or CBD flowers high in myrcene (relaxing).
Step 2: if no effect after 2–4 weeks — consult a doctor. Since August 2022 medical cannabis can be prescribed on a normal prescription in Switzerland.
FAQ
Does CBD work for chronic pain?
It can support, especially in inflammatory pain, tension and the sleep component. For clearly neuropathic pain the evidence is stronger for THC-containing cannabis.
Can CBD replace painkillers?
For mild pain often yes. It doesn't categorically replace paracetamol or ibuprofen — discuss with your doctor.
Which works faster?
THC, usually stronger and faster, especially in acute pain. CBD is subtler, with no high and little tolerance.
How much CBD for pain?
Typical starting doses: 20–40 mg/day in 2–3 doses. Adjust. For medical THC, titrate strictly per prescription.
Can I get a cannabis prescription for pain in Switzerland?
Since August 2022 yes, given a valid medical indication. Details in «cannabis prescription Switzerland».
