CBD vs medical cannabis — the honest comparison
Same plant, two worlds — legally, pharmacologically and in access. Who needs what — and why legal CBD is already enough for many.
Medical cannabis (with THC) in Switzerland has, since August 2022, required only a normal medical prescription — no more FOPH authorisation. CBD (< 1% THC) is freely sold, no prescription, no diagnosis, cheaper and instantly available. For relaxation, sleep, stress and mild pain, CBD is enough for most people. For diagnosed chronic pain, MS spasticity or chemotherapy, medical cannabis with THC is often stronger — but more expensive and tied to a medical consultation.
Legal status in Switzerland
CBD products with less than 1% THC do not fall under the Narcotics Act. Freely sold, no prescription, no diagnosis required, only age (18+) verified. It is the most liberal framework in Europe.
Medical cannabis with THC (> 1%) has been much more accessible since 1 August 2022: any licensed doctor can prescribe it on a narcotics prescription. The previously required FOPH exemption is gone. Dispensed via licensed pharmacies.
Effect profile — what differs
CBD is non-psychoactive. It modulates the endocannabinoid system, affects serotonin and vanilloid receptors, and is typically associated with relaxation, less tension and better sleep.
THC is psychoactive and binds directly to CB1 receptors. Stronger analgesic, appetite-stimulating and anti-nausea effects — but with a high. In medical use, the dose is titrated precisely, often as a full-spectrum extract or standardised flowers.
Cost — where the gap hurts
CBD flowers in a Swiss shop: 5–8 CHF/g. At CannabisClub.ch, direct from the indoor producer: from 1–2 CHF/g.
Medical cannabis in pharmacy: 10–18 CHF/g, rarely covered by basic health insurance. Reimbursement possible with medical justification and prior cost approval — but only for specific indications.
Who needs which
Stress, sleep issues without clinical diagnosis, evening relaxation, mild muscle or joint complaints: CBD is usually enough. Instantly available, legal, predictable.

Can my GP prescribe medical cannabis?
Since August 2022 — yes. Any licensed doctor in Switzerland can prescribe cannabis-based medicines on a narcotics prescription, provided there is a medical indication.
Is CBD an alternative to medical cannabis?
For mild complaints often yes. For diagnosed chronic conditions, CBD does not replace medical THC — but can complement it.
Does insurance cover medical cannabis?
Not by default. Basic insurance only reimburses in narrowly defined cases and usually after prior cost approval. Rules change; clarify before starting therapy.
Can I get CBD on prescription too?
Pure CBD does not require a prescription in Switzerland. Only THC-containing products (> 1%) are prescription-only.
Which acts stronger?
THC-containing medical cannabis is pharmacologically stronger, especially for pain. CBD is gentler but without high and freely accessible.
